  • Your complaint must be filed no later than one year from the date of the event or from awareness of the event. See the complaint admissibility requirements.
  • The only mandatory fields are those marked with an asterisk (*). For non-mandatory information, provide as many elements as possible. You can still forward your complaint although you may not have all the information.
  • Do not hesitate to request our assistance if you have any questions or problems.
