Gather evidence. The report must be accompanied by all the evidence you have in relation to the event or situation you wish to report.
Fill in the online report form yourself.
In some situations, you can also file a report orally with a member of the Commissioner's staff.

By contacting a human rights or community organization that offers assistance in filing a police ethics report.
